Thank you for your interest in this interview study. The purpose of this research is to understand the experience of adults and adolescents with von Willebrand Disease (VWD) and gather feedback on a related questionnaire. We will be asking you some questions to see if you are/your adolescent is eligible to participate. Individuals who meet the eligibility criteria may be asked to take part in an interview.

Any information you provide to us as part of the eligibility questions will be treated as confidential. It will be combined with feedback from others like yourself and will be stored in an access-controlled file. Your/your adolescent’s information will be kept secure at all times. Personally identifying information, such as names and locations, will be removed from your/your adolescent’s data and your/your adolescent’s information will not be passed to any other organization without your permission. Any personally identifying information you/your adolescent give to us as part of the eligibility questions will be destroyed after five years; data that does not contain your personally identifying information will be destroyed 15 years after study completion.

If you are/your adolescent is eligible to take part in the interview, you will be provided with a study information sheet, which explains how the study works and your/your adolescent’s rights as a study participant. You can then decide, based on the study information sheet, whether you/your adolescent would like to take part or not.
